There are two methods for performing
a rhinoplasty. In the "closed" method, the surgeon performs the
rhinoplasty within the nose. The nose heals more quickly and does
not disturb the nasal tissues as much. The "open" procedure involves
placing a small surgical incision outside between the two nostrils.
This gives the surgeon better visibility of the internal structures
of the nose, and is used in severe rhinoplasty cases.
The typical rhinoplasty surgery
requires an hour or two to perform. Furthermore, rhinoplasty may
be performed under a general anesthetic or with a local anesthetic
depending on the surgeon’s and the patient’s preference.
Following surgery, a splint is
applied and the nose is lightly packed with surgical gauze. Swelling,
bruising, puffiness, reddening, and stiffness occur and may last
for weeks. However most patients resume normal to light activity
within a few days.
During the recovery process, it
is not uncommon to experience minor bleeding, and gauze bandages
must be changed a few times daily during the first few days following
surgery. To relieve pain and discomfort, the surgeon may prescribe
pain medication. Cold compresses will reduce associated pain and
swelling.
In some rhinoplasty cases it may
take over a year before one can accurately judge the permanent outcome
of the rhinoplasty surgery due to the constant changing and healing
of the nasal tissues, blood flow, skin thickness, etc. Occasionally
a secondary rhinoplasty surgery may be required to achieve an optimal
outcome.
There are distinctive features
of ethnic noses around which has grown an entire subspecialty of
Ethnic Rhinoplasty. Fortunately for the medical traveler to Bangkok,
most plastic surgeons are experienced in the specific planning issues,
techniques, implants, and recovery that are unique to the planning
and performance of rhinoplasty in ethnic patients and patients of
color.
